Return the form along with your check or money order
payable to Toltec School District to:

The district will send you a tax receipt for your contribution.
Submit your Toltec’s tax credit receipt to your tax preparer when filing your taxes or use to
complete your Arizona State taxes with your online program.
Contributions must be received by April 15th.
